Denver, CO – Revelstoke Capital Partners (“Revelstoke”), a Denver-based private equity firm, has completed its investment in Upstream Rehabilitation, Inc. (“Upstream” or the “Company”).
Upstream is a leading national provider of a comprehensive set of outpatient rehabilitation services, including free-standing physical therapy clinics, outpatient rehabilitation management services and a member network of rehabilitation service providers. The Company is one of the largest and fastest growing in the industry with more than 300 owned and managed clinics across 23 states.

Winston & Strawn LLP served as legal advisor to Revelstoke. Harris Williams & Co. served as financial advisor, and Proskauer Rose LLP served as legal advisor, to Upstream in the transaction.
Mezzanine capital was provided by a group including Yukon Partners and Northwestern Mutual Capital and Ally Corporate Finance acted as administrative agent, sole lead arranger and book runner on the senior credit facility.
